I did something like this to a conquest server setup in our clinic so that I could query/retrieve exams using a VPN connection to the clinic's network. I used Kpacs as a viewer.

I managed to run a MySQL database and conquest server in windows 7.

I installed the following

mysql-essential-5.1.42-win32.msi
dicomserver1415

I don't have a good set of images to try out the system. I have a few TBy of backup images that i will use to regen a database.
I'll post the results and a tutorial because I nearly went nuts doing this.
